Prior to your first visit please print out the new patient forms below and have the health questionnaire and paperwork completed prior to your first visit.
Cancellation Fees/Rescheduling
We understand that sometimes schedule adjustments are necessary and due to the length of a treatment and time reserved for you, please give a 24-hour notice for cancellations and rescheduling.
Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment, and clients on the waiting list miss the opportunity to receive a treatment. Since that time is reserved for you personally a cancellation fee of $35.00 may be applied to your following visit.
If you are more than 15 minutes late it may be necessary to reschedule your appointment.
